Your entry/entries will automatically be submitted into our system when your pledge is received. To be eligible, pledge forms must by submitted to UWGLV by January 31, 2024. Winners must complete a Form W9 and will receive at 1099 for the value of the prize. Winners are responsible to pay for all applicable taxes on prizes.
